How PA's CHC program affects you

Starting in 2019, Pennsylvania’s Community HealthChoices (CHC) program will provide Medical Assistance (Medicaid) coverage to individuals who are 21 or older and have both Medicare and Medicaid, or who receive long-term services and supports (LTSS) through Medicaid because they need help with everyday personal tasks. 

If you have Medicare and Medicaid, enrolling in a CHC plan will not affect your Medicare benefits.  

If you have any questions about the CHC program, call the PA Independent Enrollment Broker at 1-844-824-3655.

If you’re a Health Partners Medicare Special (HMO SNP) plan member you can stay with HPP, keep seeing your Medicare doctor and keep receiving the benefits we provide.

What do I get with our Health Partners Medicare Special (HMO SNP) plan?

$0 or low cost doctor and specialist visits.

A yearly eye exam and an allowance for eyeglasses or contacts.

You get two cleanings and exams per year plus annual X-rays and fluoride treatment at no cost to you.

Membership to an in-network fitness center or an at-home fitness kit for free.

60 one-way trips to doctor visits and pharmacies.

Points for completing healthy activities, which can be redeemed for goods and gifts.
